Dear Students,
The time has come for the annual MMC student insurance open enrollment period. From June 20, 2016 - July 30, 2016 (all continuing students and 1st year medical and dental) and June 20, 2016 – August 20, 2016 (1st year MSPH, MHS, and PhD) have the option to enroll in the 2016 – 2017 Student Insurance Plan or waive and seek external coverage. Information about the 2016 – 2017 Student Insurance Plan is included below:
2016 - 2017 Student Insurance Rates
Because of plan design changes and health care reform, a tier was added to separate those covering one child vs. multiple children. Students with dependents are encouraged to view options through the Health Insurance Market Place via HealthCare.gov. The Office of Student Life is looking to offer an application for insurance assistance to those with dependents. More information will follow.
To ENROLL in the 2016 – 2017 Student Insurance Plan, complete the Student Insurance Enrollment Form HERE.
To WAIVE coverage in the 2016 – 2017 Student Insurance Plan, complete the Student Insurance Waiver Form HERE.

Student Insurance Concerns Form
As with any all-encompassing program, issues may arise at any time. For this reason, the Office of Student Life created the Student Insurance Concerns Form for students to report issues (i.e. billing matters, verification issues, etc.) they encounter while enrolled in the plan. The form can be viewed HERE.
By completing the Student Insurance Concerns Form, the Office of Student Life is immediately notified of all concerns, allowing us to provide prompt responses to any and all issues.
